A practical action plan for reinventing healthcare in a post-pandemic world—from a physician-entrepreneur who works with Fortune 500 companies. If the healthcare system were an emperor, Covid-19 tragically revealed that it had no clothes. Healthcare had to adapt, and quickly―sparking a dramatic acceleration of virtual care, drive-through testing, and home-based services. In the process, old rules were rewritten and, perhaps surprisingly, largely in a good way for patients. To succeed in the post-pandemic world, all of us―patients, caregivers, providers, employers, investors, technologists, and policymakers―need to understand the new healthcare landscape and change our strategies and behaviors accordingly. In Care After Covid, practicing physician and business leader Dr. Shantanu Nundy—Chief Medical Officer of Accolade, which provides technology-enabled health services to Fortune 500 companies as well as small businesses―lays out a comprehensive plan to transform healthcare along three dimensions: Distributed: healthcare will happen where health happens. It will shift from where doctors are to where patients are—at home, in the community, and increasingly on their phones. Digitally enabled: healthcare and the relationships that are central to care will be strengthened by data and technology. It will shift from being siloed to connected, from being episodic to continuous, from one-size-fits-all to more personalized. Decentralized: healthcare decisions and resources will be in the hands of those closest to care. The power to determine who gets care and how they get it will shift away from governments and insurance companies to communities, employers, doctors, and patients. Filled with firsthand insights and stories from the frontlines of healthcare—as well as innovative solutions that were proven effective before and during the pandemic—Care After Covid shows all stakeholders in the healthcare ecosystem exactly what needs to change and, more importantly, how to do it. The time to act is now. We can’t afford not to.

Almost half a century ago, policy leaders issued the Declaration of Alma Ata and embraced the promise of health for all through primary health care (PHC). That vision has inspired generations. Countries throughout the world—rich and poor—have struggled to build health systems anchored in strong PHC where they were needed most. The world has waited long enough for high-performing PHC to become more than an aspiration; it is now time to deliver. The COVID-19 (Coronavirus) pandemic has facilitated the reckoning for that shared failure—but it has also created a once-in-a-generation opportunity for transformational health system changes. The pandemic has shown policy makers and ordinary citizens why health systems matter and what happens when they fail. Bold reforms now can prepare health systems for future crises and bring goals such as universal health coverage within reach. PHC holds the key to these transformations. To fulfill that promise, however, the walk has to finally match the talk. Walking the Talk: Reimagining Primary Health Care after COVID-19 outlines how to get there. It charts an agenda to reimagined, fit-for-purpose PHC. It asks three questions about health systems reform built around PHC: Why? What? How? The characteristics of high-performing PHC are precisely those that are most critical for managing the pressures coming to bear on health systems in the post-COVID world. The challenges include future outbreaks and other emergent threats, as well as long-term structural trends that are reshaping the environments in which systems operate in noncrisis times. Walking the Talk highlights three sets of megatrends that will increasingly affect health systems in the coming decades: • Demographic and epidemiological shifts • Changes in technology • Citizens’ evolving expectations for health care. Reimagined PHC systems will be equipped through optimized system design, financing, and delivery to ensure high-quality services, care to address patients’ needs, fairness and accountability, and resilient systems.

The COVID-19 pandemic has led to radical transformations in the organisation and delivery of health and care services across the world. In many countries, policy makers have rushed to re-organise care services to meet the surge demand of COVID-19, from re-purposing existing services to creating new ‘field’ hospitals. Such strategies signal important and sweeping changes in the organisation of both ‘COVID’ and ‘non-COVID’ care, whilst asking more fundamental questions about the long-term organisation of care ‘after COVID’. In some contexts, the pandemic has exposed the fragilities and vulnerabilities of care systems, whilst in others, it has shown how services are organised to be more resilient and adaptive to unanticipated pressures. The COVID-19 pandemic presents a rare opportunity to examine empirically and to develop new theoretical frameworks on how and why health systems adapt to such unusual and intense pressures. International contributors consider how responses to COVID-19 are transforming the organisation and governance of health and care services and explore questions around strategic leadership at local, regional, national and transnational level. The book offers unique insight and analysis on the dynamics of policy-making, the organisation and governance of care organisations, the role of technologies in governing, the changing role of professionals and the possibilities for more resilient care systems.

This collection explores the changing meaning and enactments of care in teacher education in light of the COVID-19 pandemic, from preservice teachers and teacher candidates to in-service teachers and education faculty. Over fifty international teacher educators explore the complicated concept of care in different content areas, learning contexts, and communities of learners, using different conceptual frameworks and methodological orientations. Throughout, this book situates research and reflection at the nexus of teacher education, care, and COVID-19 in order to reconstruct care in post-pandemic teacher education. Timely and incisive, this collection raises important questions and offers relevant examinations to consider how post-pandemic teacher education as a field will move forward in preparing and caring for those who will, in turn, care for their future students. The book is essential reading for teacher educators, scholars, and anyone interested in the notion of care in education.

Myalgic encephalomyelitis (ME) and chronic fatigue syndrome (CFS) are serious, debilitating conditions that affect millions of people in the United States and around the world. ME/CFS can cause significant impairment and disability. Despite substantial efforts by researchers to better understand ME/CFS, there is no known cause or effective treatment. Diagnosing the disease remains a challenge, and patients often struggle with their illness for years before an identification is made. Some health care providers have been skeptical about the serious physiological - rather than psychological - nature of the illness. Once diagnosed, patients often complain of receiving hostility from their health care provider as well as being subjected to treatment strategies that exacerbate their symptoms. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome proposes new diagnostic clinical criteria for ME/CFS and a new term for the illness - systemic exertion intolerance disease(SEID). According to this report, the term myalgic encephalomyelitis does not accurately describe this illness, and the term chronic fatigue syndrome can result in trivialization and stigmatization for patients afflicted with this illness. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome stresses that SEID is a medical - not a psychiatric or psychological - illness. This report lists the major symptoms of SEID and recommends a diagnostic process. One of the report's most important conclusions is that a thorough history, physical examination, and targeted work-up are necessary and often sufficient for diagnosis. The new criteria will allow a large percentage of undiagnosed patients to receive an accurate diagnosis and appropriate care. Beyond Myalgic Encephalomyelitis/Chronic Fatigue Syndrome will be a valuable resource to promote the prompt diagnosis of patients with this complex, multisystem, and often devastating disorder; enhance public understanding; and provide a firm foundation for future improvements in diagnosis and treatment.
