In consultation with Dr. Lucky Jain, Consulting Editor, Drs. Dempsey and EL-Khuffash have chosen a wide range of topics in cardiovascular issues that cover new diagnostic techniques and important areas of management, including the role of volume administration, inotropes, and corticosteroids in the setting of cardiovascular instability. These remain critical questions in neonatal care, and authors were selected based on their expertise and ability to provide the reader with the most recent up-to-date evidence upon which to direct intervention. The clinical review articles in this issue will address the relationship between intervention and outcome in preterm hypotension, the management of cardiovascular instability in the setting of pulmonary hypertension and therapeutic hypothermia, and finally ask if it’s time for a definitive trial in the management of PDA. The final article in the issue highlights the clinical trials to date, what trials are currently ongoing, and what the future holds in cardiovascular support for the preterm infant. The reader will come away with the most current information on this topic from international experts including Europe, Canada, USA, and Australia.

The impact of cardiovascular disease on an infant extends from the fetal period to well beyond childhood. Perinatalogists and neonatologists can impact maternal and fetal health through wide range of diagnostic modalities and interventional techniques. For our edition focused on cardiovascular health, we sought to encompass the breadth of knowledge that would be the most relevant for the bed side clinician. Our goal was to assemble contents that would allow a clinician to quickly peruse the journal, and then be prepared to make a medical decision. The interaction between cardiology and perinatology/neonatology includes genetics, diagnostics, interventions, counseling, routine stabilization and day to day care. Ultimately, the goal is to establish the foundation for a healthy adult. For this reason, we have even included chapters on topics that are significant on a day to day basis (such as the proper environment for a newborn) and a long term basis (like the overall neurodevelopmental impact of our interventions). Hopefully, whether in the middle of the night as an emergency reference or during the day as a reliable guide, this edition of Clinics in perinatology will be an important bedside tool for anyone that participates in the care of a patient with perinatal heart disease.

"This book is a rare find for APRNs, especially PNPs and NNPs, and provides excellent APRN collaboration instances in everyday clinical and academic settings. The chapters are written by interprofessional healthcare providers, such as NNPs and PharmDs, and provide information and practice guidelines from a collaborative perspective. " –Holly Wei, PhD, RN, CPN, NEA-BC, FAAN Professor and Assistant Dean for PhD Program University of Louisville School of Nursing This is the first text to address fetal and neonatal pharmacology for the APN and nursing student. With a strong focus on interprofessional collaboration, it delivers core pharmacologic concepts and practice guidelines in a clear, systematic manner for ease of use. This resource reflects real-word scenarios as it discusses perinatal and intrapartum pharmacologic therapies and their implications for fetuses and neonates. It covers such postnatal pharmacology considerations as human milk and maternal drug addiction. A table including 100 of the most commonly used medications prescribed in the NICU serves as a quick reference for both academic and clinical settings. The text explores the genesis, evolution, and current knowledge surrounding pharmacologic therapies used to treat common problems afflicting preterm and critically ill neonates. Chapters, written by interprofessional teams, include illustrations and concept maps of key content—promoting active learning for visual and tactile learners, learning objectives, and discussion questions. The text meets the accreditation requirements for MSN and DNP programs offering the 3P courses to neonatal and pediatric NP students, Clinical Nurse Specialist students, and midwifery students. Key Features: Provides clear, concise descriptions of principles of fetal and neonatal pharmacology Includes illustrations and concept maps providing visual and tactile learners with tools to engage and demonstrate active learning Presents illustrations and "concept maps" of key content to foster active learning for visual and tactile learners Offers Learning Objectives and Discussion Questions in each chapter Includes a table of 100 commonly used NICU medications Provides core curricular content, written by interprofessional author teams, which aligns with the 2021 AACN Essentials and 2022 NTFS Standards for Quality Nurse Practitioner Education

This issue of Clinics in Perinatology will carry the reader through the perinatal period and examine pain management throughout that continuum. Beginning with the genetics of obstetrical pain and opioid use in pregnancy, the discussion moves to the provision of anesthesia to the mother and fetus during fetal surgery - an area of intense concern and interest in many centers. There is an extensive discussion of both pharmacologic and non-pharmacologic management of pain during delivery. A discussion of regional anesthetic techniques is increasingly relevant in light of increasing evidence of adverse neurodevelopmental consequences of fetal exposure to general anesthetics and sedatives. Pain, its implications and management, are extensively covered including discussions of how to assess neonatal pain and how best to provide sedation and non-pharmacologic pain management, systemic pharmacologic, or regional techniques. Of particular interest are the reviews of the potential neurodevelopmental impact of both the treatment and the failure to adequately treat pain in the newborn. This topic is receiving an enormous amount of attention from all those who care for children as well as government and the media.
