Complications Of Cancer Therapy Best Practices In Prevention And Management

The Department of Cardiology at The University of Texas MD Anderson Cancer Center was established on September, 1, 2000. In the past 15 years, we have evaluated and treated more than 10,000 cancer patients with cancer ther¬apy-related cardiovascular complications. Three years ago, we initiated the MD Anderson Practice (MAP) project to distillate our practice patterns into al¬gorithms to be shared with the onco-cardiology community. Because cancer is often an exclusion criterion for cardiology studies, purely evidence-based man¬agement of cancer therapy-related cardiovascular complications is not possible. With this vacuum of knowledge, various "guidelines" have proliferated that are either misleading or difficult to practice. In this manual, we present 16 MAPs that have been extensively reviewed by the cardiologists at MD Anderson. These MAPs should be considered our best practices rather than "guidelines." These MAPs will be updated frequently to reflect advances in the field. This manual con¬sists of MAPs, figures, and tables. We hope you will find these materials useful to your practice and provide us with feedback to improve these MAPs.

Adjuvant treatment is administered prior to or as follow up to surgical procedures for breast cancer. Proven success in using medical therapies allowing for breast conserving procedures or reducing risk of occurrence. Although there has been much progress towards a cure, including the introduction of new targeted therapies, metastasizing cancer remains highly incurable.

Rising health care costs are a central fiscal challenge confronting the United States. National spending on health care currently accounts for 18 percent of gross domestic product (GDP), but is anticipated to increase to 25 percent of GDP by 2037. The Bipartisan Policy Center argues that "this rapid growth in health expenditures creates an unsustainable burden on America's economy, with far-reaching consequences". These consequences include crowding out many national priorities, including investments in education, infrastructure, and research; stagnation of employee wages; and decreased international competitiveness.In spite of health care costs that far exceed those of other countries, health outcomes in the United States are not considerably better. With the goal of ensuring that patients have access to high-quality, affordable cancer care, the Institute of Medicine's (IOM's) National Cancer Policy Forum convened a public workshop, Delivering Affordable Cancer Care in the 21st Century, October 8-9, 2012, in Washington, DC. Pericardial disease is a broad term that describes a wide range of pathologies. The clinical aspects of pericardial disease encompass acute pericarditis, pericardial tamponade, pericardial effusion, constrictive pericarditis, and effusive-constrictive pericarditis. Those disorders differ not only in clinical presentation but also in the timeline of disease development; for example, pericardial tamponade is commonly an acute, life-threatening event, whereas constrictive pericarditis is a chronic process developing over months to years. Therefore, pericardial disease management is challenging for most clinicians. The evidence base in the field is relatively scarce compared with other disease entities in cardiology. European Society of Cardiology released guidelines for the diagnosis and management of pericardial diseases. Currently there are no guidelines from American cardiology societies to help clinicians in dealing with pericardial disease. In this book, my goal is to provide extensive review of pericardial disease evaluation and management. A unified, stepwise pathway-based approach for the management of pericardial disease is provided at the end of the book.
