Countering Biological Threats

In response to a request from the U.S. Congress, this book examines how the unique experience and extensive capabilities of the Department of Defense (DOD) can be extended to reduce the threat of bioterrorism within developing countries outside the former Soviet Union (FSU). During the past 12 years, DOD has invested $800 million in reducing the risk from bioterrorism with roots in the states of the FSU. The program's accomplishments are many fold. The risk of bioterrorism in other countries is too great for DOD not to be among the leaders in addressing threats beyond the FSU. Taking into account possible sensitivities about a U.S. military presence, DOD should engage interested governments in about ten developing countries outside the FSU in biological threat reduction programs during the next five years. Whenever possible, DOD should partner with other organizations that have well established humanitarian reputations in the countries of interest. For example, the U.S. Agency for International Development, the Centers for Disease Control and Prevention, and the World Health Organization should be considered as potential partners.

The attacks of September 11 and the release of anthrax spores revealed enormous vulnerabilities in the U.S. public-health infrastructure and suggested similar vulnerabilities in the agricultural infrastructure as well. The traditional public health response-surveillance (intelligence), prevention, detection, response, recovery, and attribution-is the paradigm for the national response not only to all forms of terrorism but also to emerging infectious diseases. Thus, investments in research on bioterrorism will have enormous potential for application in the detection, prevention, and treatment of emerging infectious diseases that also are unpredictable and against which we must be prepared. The deciphering of the human genome sequence and the complete elucidation of numerous pathogen genomes, our rapidly increasing understanding of the molecular mechanisms of pathogenesis and of immune responses, and new strategies for designing drugs and vaccines all offer unprecedented opportunities to use science to counter bioterrorist threats. But these same developments also allow science to be misused to create new agents of mass destruction. Hence the effort to confront bioterrorism must be a global one. Countering Bioterrorism makes the following recommendations: Recommendation 1: All agencies with responsibility for homeland security should work together to establish stronger and more meaningful working ties between the intelligence, S&T, and public health communities. Recommendation 2: Federal agencies should work cooperatively and in collaboration with industry to develop and evaluate rapid, sensitive, and specific early-detection technologies. Recommendation 3: Create a global network for detection and surveillance, making use of computerized methods for real-time reporting and analysis to rapidly detect new patterns of disease locally, nationally, and ultimately- internationally. The use of high-throughput methodologies that are being increasingly utilized in modern biological research should be an important component of this expanded and highly automated surveillance strategy. Recommendation 4: Use knowledge of complex biological patterns and high-throughput laboratory automation to classify and diagnose infections in patients in primary care settings. Recommendation 5: USDA should create an agency for control and prevention of plant disease. This agency should have the capabilities necessary to deal effectively with biothreats.

Biosecurity and Bioterrorism, Second Edition, takes a holistic approach to biosecurity, with coverage of pathogens, prevention, and response methodology. We are experiencing an unparalleled period of advancement and innovation in the life sciences globally that continues to transform our way of life. Whether augmenting our ability to provide health care and protect the environment, or expanding our capacity for energy and agricultural production towards global sustainability, continued research and development in the life sciences is essential to a brighter future for all people. The beneficial nature of life science research is reflected in the widespread manner in which it occurs. From cutting-edge academic institutes, to industrial research centers, to private laboratories in basements and garages, progress is increasingly driven by innovation and open access to the insights and materials needed to advance individual initiatives. We must support the ongoing revolution in the life sciences by seeking to ensure that resulting discoveries and their applications, used solely for peaceful and beneficial purposes, are globally available. At the same time, we must be mindful of the risks throughout history posed by those who sought to misuse the products of new technologies for harmful purposes. Specifically, we must reduce the risk that misuse of the life sciences could result in the deliberate or inadvertent release of biological material in a manner that sickens or kills people, animals, or plants, or renders unusable critical resources. The effective dissemination of a lethal biological agent within an unprotected population could place at risk the lives of hundreds of thousands of people. The unmitigated consequences of such an event could overwhelm our public health capabilities, potentially causing an untold number of deaths. The economic cost could exceed one trillion dollars for each such incident. In addition, there could be significant societal and political consequences that would derive from the incident's direct impact on our way of life and the public's trust in government. Since 2001, the United States Government has significantly expanded its efforts to improve the Nation's ability to recognize and respond to acts of bioterrorism or other significant outbreaks of infectious disease; however, efforts targeted to prevent such threats have received comparatively limited policy focus or substantive guidance at the National level. Although it is entirely feasible to mitigate the impact of even a large-scale biological attack upon a city's population, doing so incurs a significant cost and effort. We therefore need to place increased priority on actions to further reduce the likelihood that such an attack might occur. This Strategy will guide our efforts to prevent such incidents by reducing the risk that misuse of the life sciences or derivative materials, techniques, or expertise will result in the use or intent to use biological agents to cause harm. It also complements existing policies, plans, and preparations to advance our ability to respond to public health crises of natural, accidental, or deliberate origin.

This book summarises the lectures presented at the Centre of Excellence - Defence Against Terrorism (COE-DAT) workshop on Bioterrorism of November 2008. The contributors are a diverse group of academics and practitioners, selected for their expertise in the field. Their contributions cover the definition and classification of bioterrorism and take account of its various dimensions, examining the theoretical, historical and practical aspects, as well as the defence against it. Consisting of seven papers and four summaries, the book covers subjects such as biodiversity, the historical use of biological agents and the concern for public safety, the role of the International Science & Technology Center in countering bioterrorism, the Global Forum on Biorisks, threat assessment, animal health and disease with reference to biological safety and the Biological Weapons Convention. The workshop itself was of necessity restricted to a small number of participants, but with this book, research, experience and perspectives on biological risks can be shared with a wider audience, allowing further consideration and improvements in countering risks and responding to biological threats and attacks. Inaugurated in 2005, COE-DAT is a NATO accredited Centre of Excellence; a unique centre dedicated to Defence Against Terrorism, which provides DAT training and education at strategic and operational levels and contributes to research efforts. This book examines the recent intersection of national security and public health regarding biological threats to the U.S. populace and proposes improvements to the executive and legislative development of U.S. policy addressing biological threat mitigation. Over the last 20 years, the national security community has engaged with disease-related issues that have traditionally been the scope of public health agencies. The federal government's response has been to create a single national biodefense strategy, which has been largely ineffective in improving conditions due to poor terminology, a lack of leadership, and a failure to assess government programs. Applying a public policy framework, Albert J. Mauroni examines how the government addresses biological threats-including disease prevention, bioterrorism response, military biodefense, biosurety, and agricultural biosecurity and food safety. He proposes a new approach to countering biological threats, arguing that lead agencies should focus on implementing discrete portfolios with annual assessments against clear and achievable objectives.
