The Health Transition Fund, a joint federal, provincial, & territorial effort to encourage & support evidence-based decision-making in health care reform, funded about 140 pilot projects and evaluation studies over 1997-2001 in nine theme areas. This report summarizes key learnings from 41 projects in the Fund's integrated service delivery (ISD) theme area. Introductory sections set the context for the projects (including a vision for ISD & ongoing ISD initiatives in Canada) and provide an overview of the Fund's ISD studies. The report then discusses significant & relevant findings of projects that examined clinical integration, information management & technology, and vertical integration in communities. Finally, implications of the findings for policy & practice are discussed. The appendix summarizes information on the Fund projects relevant to ISD.
