This book is for anyone with an interest in paranoid schizophrenia. It gives an outline of the author's life prior to him having schizophrenia. Then it gives details of his three very severe bouts of illness with this devastating mental illness. It also details his periods of being well, the psychiatric treatment that he received at hospital and his efforts to stay well on his prescribed medication. The book is an effort to describe the career of a paranoid schizophrenic patient, loosely written in the style of an academic case study on this subject. The book describes the voices, hallucinations and paranoid delusions that are characteristic of paranoid schizophrenia. It also offers some hope that those with this mental illness can recover from their periods of schizophrenia, and live relatively normal lives, as long as they continue to take their medication, which is an essential part of any recovery programme. The book can contribute to an understanding of paranoid schizophrenia among mental health professionals and sufferers alike.

Delusional disorder, once termed paranoia, was an important diagnosis in the late nineteenth and early twentieth centuries, and only in 1987 was it reintroduced into modern psychiatric diagnosis after being subsumed with schizophrenia. This book provides a comprehensive review of delusional disorder for psychiatrists and other clinicians. Beginning with the emergence of the concept of delusional disorder, the book goes on to detail its manifold presentations, differential diagnosis and treatment. Many instructive case histories are provided, illustrating manifestations of the various subtypes of delusional disorder, and related conditions in the paranoid spectrum. This is the most wide-ranging and authoritative text on the subject to have appeared for many years, and the first to suggest, based on the author's extensive experience, that the category of delusional disorder should contain not one but several conditions. It also emphasizes that, contrary to traditional belief, delusional disorder is a treatable illness.

There is a curious parallel between the philosophy of science and psychiatric theory. The so-called demarcation question, which has exercised philosophers of science over the last decades, posed the problem of distinguishing science proper from non-science - in par ticular, from metaphysics, from pseudo-science, from the non rational or irrational, or from the untestable or the empirically meaningless. In psychiatric theory, the demarcation question appears as a problem of distinguishing the sane from the insane, the well from the mentally ill. The parallelism is interesting when the criteria for what fails to be scientific are seen to be congruent with the criteria which define those psychoses which are marked by cognitive failure. In this book Dr Yehuda Fried and Professor Joseph Agassi - a practicing psychiatrist and a philosopher of science, respectivel- focus on an extreme case of psychosis - paranoia - as an essentially intellectual disorder: that is, as one in which there is a systematic and chronic delusion which is sustained by logical means. They write: "Paranoia is an extreme case by the very fact that paranoia is by definition a quirk of the intellectual apparatus, a logical delusion. " (p. 2.
