One reason for failure to cure solid tumors by surgery appears to be the impossibility of controlling metastases that are present but latent at the time of operation. This failure is a common clinical experience with aggressive neoplasms. but it is not always appreciated in tumors with longer survival times. e. g .• breast and colon cancer. In addition. recent evidence indicates that after resection of a primary tumor micrometas tases from it might be enhanced by suppression of immune and reticu loendothelial functions of the host. Other factors, such as increase of coagulability and stress in the perioperative period, can also promote tumor growth. The development of new metastases might be facilitated by cells forced into the circulation during operative manipulations. Such events could be important for the outcome of treatment and it is suggested that preventive measures should be directed to this systemic component of solid tumors. Radical surgery can reduce the number of tumor cells to a subclinical 3 6 stage (10 to 10 cells) in which chemotherapy might be more effective than in advanced stages. Chemotherapy, on the other hand, might aggravate the surgical morbidity by influencing the wound healing pro cess, by decreasing the immune response, and/or by toxicity to the bone marrow and to the gastrointestinal tract, for example.

This text provides a comprehensive textbook summarizing the presentation, workup, and surgical solutions for common emergencies in the cancer patient. The use of medications (antibiotics, hormones, and biologic therapies) and interventional radiologic procedures as adjuncts or as replacements for the surgical solutions are discussed. The text covers emergencies that result as adverse effects for the systemic and local treatments for cancer, emergencies that result from mechanical issues with the cancer and those that result from surviving major extirpative surgery. Diagnosis and treatment of patients' immune-compromised or thrombocytopenic status from chemotherapy is discussed, as is surgical treatment in patients with prior complex surgical therapy or radiation treatment. Finally, the text covers alternatives to surgery, including new interventional radiologic and endoscopic procedures. Surgical Emergencies in the Cancer Patient will be of great value to healthcare professionals at all levels who are involved in the treatment of emergencies in the cancer patient. It is meant for a wide group of individuals, including medical students, residents in surgery and internal medicine, fellows in surgery and oncology, and practicing oncologists and surgeons. This work is meant to be a textbook for the student, a guide for the practitioner, and a reference for the general oncologist.

This book covers all essential evidence on perioperative care and anesthetic concerns for cancer surgeries, including the evaluation and assessment of patients’ schedule for cancer surgeries, perioperative anesthetic management of various cancers, perioperative analgesia for cancer surgeries, and issues related to inoperable cancers. It reviews anesthetic considerations of all cancer types as there is a wide variation in anesthetic requirements in this subset of patients. Cancer surgeries are on the rise worldwide and healthcare facilities with cancer as a core specialty are emerging quickly around the globe. As cancer is on rise and efficient surgical management is available, more and more patients are being treated surgically. However, cancer surgeries are complicated, have varied concerns and these are not covered sufficiently in presently available textbooks of anesthesiology. This book fulfills the growing need for complete and comprehensive textbook of onco-anesthesia and fills the gap in the current texts that do not exclusively cover anesthesia for cancer surgeries. This textbook serves as a comprehensive but quick guide for trainee residents/fellows and practicing anesthesiologists, clinicians and surgeons.

This book covers all topics related to anesthetic and perioperative management, critical care and pain in cancer patients. The chapters of the book describe the principles and practices of onco-anesthesia, onco-critical care and cancer pain in great details. Cancer surgeries (onco-surgeries) are organ specific now with each surgery having its own set of anesthetic requirements and challenges. The perioperative management of these different organ-specific surgeries have been discussed based on recent evidence. The chapters have been written by experts from different cancer centers of India. Various courses like DM Onco-anesthesia and fellowship in onco-anesthesia are running in different institutes. FNB in Onco-anesthesia has been recognised this year. This book will help all those students pursuing these courses both as textbook and as reference book. This book will also be very helpful for MD/DNB students for their examination purpose as a good number of topics in their syllabus are on perioperative management of cancer patients. Moreover, anesthesiologists, who are practicing anesthesia in cancer patients, will also be benefited from this book.
