This book documents the most important clinicopathological features of rare thyroid tumours with an emphasis (and ample documentation) on their histological and immunohistochemical aspects. Whenever necessary, namely for differential diagnosis and/or therapy purposes, molecular data are also highlighted. In order to understand the histogenesis of rare lesions and to be able to make a differential diagnosis, the editors use the WHO book for endocrine tumours (in preparation) as a framework for classifying thyroid tumours. In addition to characterizing the main histotypes of thyroid tumours, this book also provides detailed information on how to approach the diagnosis of tumours according to the major morphological/immunohistochemical characteristics, the predominant morphology and whether or not they are hereditary. This book is a valuable learning tool for those involved in the differential diagnosis of thyroid lesions.

Leonard Wartofsky, MD, past president of the American Thyroid Association, and a panel of world-class clinicians critically review all aspects of thyroid cancer from pathogenesis to prognosis, including management and treatment. Organized for rapid access by busy physicians, the book covers both differentiated tumors (papillary carcinoma and follicular carcinoma) and undifferentiated cancers (anaplastic carcinoma, lymphoma, and medullary carcinoma), as well as miscellaneous and unusual cancers of the thyroid. For each tumor type the authors succinctly discuss the key aspects of their clinical presentation, cytology, diagnosis, nuclear isotope scanning evaluation, imaging by MRI or CT, monitoring, surgery, pathology, post-operative care, and prognosis. Authoritative and evidenced-based, Thyroid Cancer: A Comprehensive Guide to Clinical Management provides in one place everything that physicians treating thyroid cancer patients need to know.

Thyroid cancer can be either common or rare, and takes several forms in multiple populations, such as children and adolescents. The chapters in this book, which provide state-of-the-art knowledge in understanding and treating the condition, are therefore welcome. Our understanding of thyroid cancer is advanced by several chapters on its incidence in a Spanish population, the functionality of p53 and the use of microarray technology in research. Diagnostic issues include the roles of glycosylation and glycoproteins, and use of a γ-camera to evaluate the in vivo biodistributions and internal medical dosimetries of Iodine-131. The majority of chapters address the range of possible treatment protocols, such as IGF signalling, surgical management, sentinel lymph node biopsy, radioiodine treatment and differentiation therapy.

This atlas is the offspring of the “The National Cancer Institute (NCI) Thyroid Fine Needle Aspiration (FNA) State of the Science Conference,” hosted by the NCI and organized by Dr. Andrea Abati. Preparations for the conference began 18 months earlier with the designation of a steering committee and the establishment of a dedicated, p- manent web site. The meeting took place on October 22 and 23, 2007 in Bethesda, Maryland and was co-moderated by Susan J. Mandel and Edmund S. Cibas. The discussions and conclusions regarding terminology and morphologic criteria 1, 2 from the meeting were summarized in publications by Baloch et al. and form the framework for this atlas. The atlas is organized by the general categories of “Nondiagnostic,” “Benign,” “Follicular Neoplasm/Suspicious for a Follicular Neoplasm”, “Suspicious for Malignancy,” and “Malignant,” and it includes the defi- tions and morphologic criteria of these categories as set forth by Baloch et al. The majority of the conference participants also agreed on a category of “undetermined significance,” which is incorporated in this atlas (Chap. 4). It is critical that the cytopathologist communicate thyroid FNA interpretations to the referring physician in terms that are succinct, unambiguous, and helpful clinically. We recognize that the terminology used here is a flexible framework that can be modified by individual laboratories to meet the needs of their providers and the patients they serve.

This third edition manual provides a comprehensive, insightful, evidenced-based review of general surgical oncology and serves as a valuable resource for general surgeons in training, practicing general surgeons, and surgical oncologists. The volume describes a multidisciplinary approach that integrates clinical, radiologic, and pathologic data in formulating practical clinical management, and offers a practical approach to the most common situations when treating cancer patients. Each chapter focuses on an individual malignancy and describes the presentation of the malignancy, integrated management based on stage, landmark trials, and suggestions for who to discuss at multidisciplinary cancer conferences. Multiple tables in each chapter provide a concise yet comprehensive summary of the current status of the field. Clinical “pearls” or tips and tricks from high volume surgeons at the University of Toronto are also discussed. The third edition of the Surgical Oncology Manual will serve as a critical resource for general surgeons in training and practicing surgeons dealing with this challenging field.

This book is intended as a reference manual that will provide the busy clinician with up-to-date information on the diagnosis and treatment of uncommon and rare gynecological cancers. While standard textbooks briefly cover these tumors, this is intended as a more comprehensive yet easy-to-use guide. After opening chapters on epidemiology, pathology, and diagnostic imaging, the full range of infrequently encountered gynecological cancers (ovarian, uterine, cervical, vaginal, and vulval) is presented and discussed with the aid of high-quality illustrations. In each case, detailed attention is paid to both differential diagnosis and current treatment options. The book has been written by an international panel of experts and is the first to gather all the uncommon and rare gynecological cancers together within one volume.

Medullary Thyroid Carcinoma is a rare thyroid tumor originating from neural crest cells, the C-cell that implies several special features, secretion of different neuroendocrine markers, association with other endocrine tumors and familial appearance. The familial variant of medullary thyroid carcinoma allows an early detection by biochemical and genetic testing. At that stage acceptable surgical treatment will cure the patient. The sporadic form of medullary thyroid carcinoma will be diagnosed in a progressive stage, but adequate surgical procedure can cure these patients too. Long term survival depends on stage, age, sex, and variant of the disease and seems to be as good as in other differentiated thyroid cancers.
