Unlike most Asian and Latin American countries, sub-Saharan Africa has seen both an increase in population growth rates and a weakening of traditional patterns of child-spacing since the 1960s. It is tempting to conclude that sub-Saharan countries have simply not reached adequate levels of income, education, and urbanization for a fertility decline to occur. This book argues, however, that such a socioeconomic threshold hypothesis will not provide an adequate basis for comparison. These authors take the view that any reproductive regime is also anchored to a broader pattern of social organization, including the prevailing modes of production, rules of exchange, patterns of religious systems, kinship structure, division of labor, and gender roles. They link the characteristic features of the African reproductive regime with regard to nuptiality, polygyny, breastfeeding, postpartum abstinence, sterility, and child-fostering to other specifically African characteristics of social organization and culture. Substantial attention is paid to the heterogeneity that prevails among sub-Saharan societies and considerable use is made, therefore, of interethnic comparisons. As a result the book goes considerably beyond mere demographic description and builds bridges between demography and anthropology or sociology. Perhaps no region matches sub-Saharan Africa in its social and cultural diversity. In this collection of micro-demographic studies, the authors examine how the diversity of this region influences reproductive behavior. The empirical studies, distributed throughout the continent, are localized, in-depth studies that give special attention to contextual effects of social structure and social organization. A diverse range of topics is addressed including, adolescent sexuality and the effects of early childbearing on later fertility, the impact of development programs on fertility and the association between social organization, social diffusion, and reproductive regime. Highly revealing of the determinants of reproduction in Africa, these studies serve as a model for a new mode of demographic research. The chapters are arranged by geographical regions of the continent, with an introductory chapter outlining the editors' vision of a micro-demographic enterprise and a concluding chapter placing the African fertility transition in the context of the global fertility transition. Adolescent fertility tends to be valued and sanctioned in the countries of sub-Saharan Africa when parents have had adequate ritual or training preparation for adulthood and the child has a recognized father. Young women and adolescents who conceive and bear children within this context are widely accepted by society; those who conceive outside of marriage, however, are strongly condemned by society. Over the past 2-3 decades, most African countries have successfully raised their levels of education. Girls and women are increasingly privy to formal school education and training in trade apprenticeships, domestic service, and ritual initiation which had otherwise been denied in the past. These factors, combined with declining menarche in a few areas, and changing economic opportunities, law, and religion make it more difficult to define the exact date of entry into marriage. Many girls are taking advantage of these changing circumstances and their opportunities to obtain educations and resist early marriage and cildbearing. While defying the traditional entry into early marriage, many young women do not, however, refrain from engaging in sexual activities. Pregnancies to unwed mothers are thereby on the rise and may constitute the most profound change observed in the social context of adolescent fertility on the continent. Once pregnant, many women find themselves shut out by family planning programs and prenatal clinics which serve only married women. This paper ultimately concludes that the social context of adolescent childbearing has an effect on the outcome for mother and child which is as important as the physiological maturity of the mother.

This book focuses on reproductive health rights and assisted reproductive technologies (ART) in sub-Saharan Africa. Each chapter is connected to the other by focusing on different aspects of ART as a means of achieving conception. Topics such as regulation of ART practices, surrogacy and specific aspects of ART, which are gradually becoming acceptable but largely unregulated in Africa, promises to be of interest to scholars, researchers and fertility practitioners. Research in the book take a rights based approach and ethical analysis of ART practice in sub-Saharan Africa by authors from diverse backgrounds bringing together law and society perspectives. Readers stand to gain new knowledge on the societal, legal, medical and psychological requirements, effects and challenges of reproductive health rights and ART in the African context.
