The Pathophysiology and Pharmacotherapy of Myocardial Infarction deals with the advances in the pathophysiology and pharmacotherapy of acute myocardial infarction and related complications. This book covers two major consequences—cardiac muscle dysfunction and cardiac electrical dysfunction. This text begins by reviewing the controversy surrounding the pathogenetic mechanisms of atheroma formation and its potential reversibility, including the status of coronary risk factors and benefits of primary prevention. The pathophysiologic mechanisms of pump failure and the current approach to its management are then deliberated, followed by a discussion on the rational use of vasodilators in post-infarction heart failure and pharmacotherapy of cardiogenic shock. The rationale for the interest in the role of coronary vasospasm and thrombolytic therapy for acute myocardial infarction is also covered. This publication concludes by reviewing the approaches for the management of post-infarction ventricular tachyarrhythmias and concept of secondary prevention of sudden cardiac death. This compilation is valuable to medical students, primary care physicians, and cardiovascular specialists.

Ischemie heart disease is still the most frequent cause of death in the western world. There have been significant achievements in diagnostic procedures as well as in the medical, invasive, and surgiealtreatment ofischemieheart disease inrecent years. A variety ofdrugs are availablefor the pharmacotherapyofischemieheart disease, par ticularly nitrates, ß-blockers, and calcium-antagonists which are used as mono therapy or in various combinations. However, the selection of patients for a certain treatment, as weIl as the optimization of an individual treatment are still largely empirical. On the other hand, the recent advances in experimental cardiology emphasize the extremely complex and dynamie scenario of ischemic heart disease, involving endothelial damage, coagulation processes, metabolie and morphologie derangements, coronary constrictor mechanisms, blood flow redistribution, arrhythmogenesis, contraetile dysfunction during ischemia and reperfusion, and finally lack or presence of pain perception. Therefore, it appears desirable to close the gap between experimental and clinical cardiology and, thus, to provide a pathophysiologieal basis for rational cliniealdecisions with respect to diagnostic and therapeutic procedures. The idea for this book arose during the preparation of a seminar series on experi mental cardiology, when I found it diffieultto collect the pertinentinformation from textbooks of cardiology, physiology, pathology, and pharmacology, as well as from numerous review and original artieies on specifictopies. I am now very grateful that expert cliniealand experimental colleagues from around the world have joined me in the effort to provide a comprehensive textbook on the pathophysiology of myocar dial ischemia and its rational pharmacotherapy.

The present book covers the basic principles of cardiovascular physiology, pathophysiology and advanced pharmacology with particular emphasis on cellular mechanisms of drug action. It provides an update on the progress made in several aspects of cardiovascular diseases so that it might kindle scientists and clinicians alike in furthering basic and translational research. In addition, the book is expected to fill imperative gaps in understanding and optimally treating cardiovascular disease.

This book is open access under a CC BY 4.0 license. This quick-reference handbook offers a concise and practical review of key aspects of the treatment of ST-segment elevation myocardial infarction (STEMI) in the era of primary percutaneous coronary intervention (PPCI). In the context of STEMI, PPCI is the preferred mode of emergency revascularization. Access to PPCI is rapidly increasing and is now routinely practiced in both general and specialist hospitals and there has been a recent emphasis on developing STEMI networks to enhance and expedite the referral pathway. This coupled with concurrent developments to enhance the safety and efficacy of the PPCI procedure has heralded an era where STEMI interventions are increasingly considered an important subspecialty within interventional cardiology. Written by leading cardiologists who have been instrumental in the adoption of PPCI in their respective institutions, the book provides junior and senior cardiologists alike with insightful and thought-provoking tips and tricks to enhance the success of PPCI procedures, which may in turn translate into direct improvements in outcomes. The book is also relevant for healthcare providers and emergency department physicians.

Coronary artery disease is the leading cause of morbidity and mortality in the Western World. The knowledge base about acute myocardial infarction continues to accrue rapidly with information about pathophysiology, epidemiology, reperfusion therapy, as well as adjunctive treatment strategies. There are however areas where substantial more investigation is required. These include among others 1) patient education strategies for earlier recognition of symptoms which would facilitate earlier and more effective therapy, 2) means to maximize myocardial flow rather than just epicardial flow very early on, 3) an approach to improve myocardial salvage with either adjunctive approaches which are pharmacologic or mechanical, 4) secondary prevention after the initial event including the need for testing and treatment with multiple antiplatelet strategies, 5) ways to define and delivery optimal care with patients with acute myocardial infarction, and 6) new approaches to myocardial cell regeneration therapy. Approaches to these issues will define the future of the treatment of acute myocardial infarction. In this issue of the Medical Clinics of North America, Guest Edited by luminaries David Holmes and Mandeep Singh, we explore these areas and define what works, what has not worked, and what may work in the future.
