Two critical mishap program documents, one for the USAF and one for the Air National Guard, are reproduced here. This instruction implements Air Force Policy Directive (AFPD) 91-2, Safety Programs. It establishes mishap prevention program requirements, assigns responsibilities for program elements and contains program management information. The Air Force System Safety Handbook was prepared as a resource document for program office system safety managers and system safety engineers. It is not designed to answer every question on the topic of system safety nor is it a cookbook that guarantees success.
