Current Advances In Breast Cancer Research

Breast cancer is a recognized disease around the world with varying patient outcomes based on the type of breast cancer, access to healthcare and other factors. Survival rates for breast cancer are significantly lower in metastatic cases than localized cases. Early diagnosis and effective treatments for the efficient management of breast cancer are now in demand , as they help to prolong patient life. There have been many breakthrough developments in the molecular biology of breast cancer research in recent times. Advancements in diagnostic techniques (imaging and biomarker detection) for breast cancer have improved the screening of the disease and have improved patient outcomes. Despite these enhancements, the disease is still lethal for patients and the search for a cure requires a complete understanding of the disease. This book describes recent advances in translational research in breast cancer and presents emerging applications of this research that promise to have meaningful impacts on diagnosis and treatment. It introduces ideas and materials derived from the clinic that have been brought to "the bench" for basic research, as well as findings that have been applied back to "the bedside". Detailed attention is devoted to breast cancer biology and cell signaling pathways and to cancer stem cell and tumor heterogeneity in breast cancer. Various patient-derived research models are discussed, and a further focus is the role of biomarkers in precision medicine for breast cancer patients. Next-generation clinical research receives detailed attention, addressing the increasingly important role of big data in breast cancer research and a wide range of other emerging developments. An entire section is also devoted to the management of women with high-risk breast cancer. Translational Research in Breast Cancer will help clinicians and scientists to optimize their collaboration in order to achieve the common goal of conquering breast cancer.

X-ray mammography screening is the current mainstay for early breast cancer detection. It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

In an effort to successfully treat breast cancer, it is imperative to a) fully understand the disease with all its heterogeneity, b) understand the factors that influence the metastasis of breast cancer to distant organs making it lethal and c) understand the underlying processes that lead to the phenomenon of drug-resistance making the disease particularly incurable. The book explores all of these issues, including the phenomenon of epithelial-mesenchymal-transition, cancer stem cells as well as microRNAs in an attempt to better understand the disease in connection to its heterogeneity/metastasis/drug-resistance as well as to propose novel signaling pathways for therapeutic intervention. The profiling of tumors to molecularly classify breast cancers is also investigated so that customized targeted therapies can be developed. ​
