As hospital costs escalate and gynaecologists become more skilled at non-invasive techniques, a growing number of diagnostic and therapeutic procedures can be carried out in the office under short-acting or local anesthesia. The second edition of this handbook for practicing gynaecologists reflects this change, and covers all diagnostic and therapeutic procedures possible in the office setting. This is greatly to the patient's advantage from a psychological as well as an economical and logistical standpoint. The important advances here in terms of new equipment and techniques include laparoscopy for sterilization and biopsy, the use of ultrasound in pregnancy termination, to name a few. Other diagnostic procedures (breast, vulvar and vaginal biopsies; urethrocystoscopy; and endometrial biospy) and other local therapeutic maneuvers (eg, conization) can also be performed in the office, as well as minor procedures performed in the first trimester of pregnancy (eg, amniocentesis).
