Les difficultés à interagir dans le monde social et à s'y adapter sont une plainte centrale des personnes souffrant de troubles psychiatriques et notamment de schizophrénie ou de troubles apparentés. Ces difficultés, qui constituent un frein au processus de réhabilitation psychosociale, pourraient en partie être expliquées par un dysfonctionnement des processus regroupés sous le terme de cognition sociale. La cognition sociale est définie comme la capacité à construire des représentations sur soi-même et autrui, et sur les relations entre soi et les autres, ainsi qu'à utiliser ces représentations de manière flexible, afin de guider le comportement social ; et inclut des processus tels que le traitement des émotions, la théorie de l'esprit, le style attributionnel, la perception et les connaissances sociales. Des déficits concernant les différentes composantes de la cognition sociale ont été largement mis en évidence dans la schizophrénie et les troubles apparentés, et semblent par ailleurs avoir un lien fort avec les difficultés observées dans les domaines du fonctionnement social, mais également des relations privilégiées avec les processus neurocognitifs, même si ces deux composantes semblent relativement indépendantes. Afin de compenser ces troubles, de nombreuses prises en charge de remédiation cognitive ont été proposées ces dernières années. L'objectif de ce travail de thèse a été d'évaluer la possibilité d'améliorer les déficits touchant la sphère de la cognition sociale des personnes souffrant de troubles psychotiques à l'aide d'un outil de remédiation spécifiquement développé à cet effet, le programme RC2S. Etant donnée l'hétérogénéité des déficits touchant la sphère de la cognition sociale dans la schizophrénie, et l'importance fondamentale portée au transfert des compétences dans la vie quotidienne, RC2S a été développé comme une thérapie individualisée et flexible, qui permet aux patients de s'entraîner aux interactions sociales dans un environnement réaliste, et d'adapter la prise en charge aux difficultés spécifiques de la personne. Nous présenterons ici trois études basées sur la méthodologie du cas unique, mettant en évidence l'impact de cette thérapie sur les troubles de la cognition sociale de deux patients souffrant de schizophrénie et d'un patient présentant un trouble de la personnalité schizoïde.

La remédiation cognitive est devenue en quelques années un outil thérapeutique important dans le domaine des maladies psychiatriques, et plus particulièrement dans celui des troubles schizophréniques. Parmi les programmes utilisés, RECOS est l'un des seuls à proposer un entraînement qui tient compte du profil cognitif individuel, permettant ainsi de répondre de manière ciblée à la grande hétérogénéité des déficits observés. Cet ouvrage constitue le support de base indispensable à la formation délivrée aux futurs thérapeutes RECOS. Il se divise en deux parties. La première partie présente les données scientifiques actuelles sur les troubles cognitifs de la schizophrénie et les moyens d'y remédier. Le lien entre les performances cognitives et les capacités fonctionnelles permet de comprendre comment et pourquoi la remédiation cognitive favorise la réinsertion sociale et professionnelle. La deuxième partie fait office de manuel d'utilisation pour tous les thérapeutes (psychologues, psychiatres, infirmiers, ergothérapeutes) souhaitant utiliser RECOS. L'ouvrage décrit les pathologies psychiatriques visées par le programme, la schizophrénie n'étant pas la seule concernée. Il aborde ensuite les différentes étapes du traitement, en consacrant une place importante à l'évaluation cognitive et clinique ainsi qu'aux exercices de remédiation. Des cas cliniques illustrent la manière d'adapter le travail thérapeutique au profil cognitif de chaque participant. Afin que le lecteur puisse bénéficier d'un maximum d'informations et de documents pratiques, plusieurs outils nécessaires à l'utilisation du programme figurent en annexe de l'ouvrage. La cognition sociale recouvre l'ensemble des processus de traitement de l'information permettant de comprendre les autres et d'interagir avec eux. L'altération de ces processus dans la schizophrénie a pour conséquence d'importantes difficultés relationnelles pour les personnes qui en souffrent. Les conséquences de ces difficultés sur l'insertion - aussi bien sociale que professionnelle – et, plus globalement, sur le pronostic de la maladie peuvent être majeures. Il est donc très important de prendre en considération cette dimension de la schizophrénie dans l'évaluation et la prise en charge. Cet ouvrage considère la cognition sociale d'un point de vue d'abord théorique, puis pratique. La lecture intentionnelle, le traitement des informations faciales, la théorie de l'esprit, l'empathie, le raisonnement et la perception sociale sont explicités et la manière de les évaluer est envisagée. Les outils de remédiation cognitive destinés à agir sur la cognition sociale sont ensuite présentés. Mieux comprendre la cognition sociale permet de mieux prendre en charge ses troubles. Plusieurs programmes consacrés à la remédiation cognitive sont disponibles en français, dont RC2S, Gaïa, ToMRemed et SCIT. Ces programmes sont présentés de manière pratique dans leurs indications et leurs modalités d'utilisation.

Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?

How do our patients come to be the way they are? What forces shape their conscious and unconscious thoughts and feelings? How can we use this information to best help them? Constructing psychodynamic formulations is one of the best ways for mental health professionals to answer questions like these. It can help clinicians in all mental health setting understand their patients, set treatment goals, choose therapeutic strategies, construct meaningful interventions and conduct treatment. Despite the centrality of psychodynamic formulation to our work with patients, few students are taught how to construct them in a clear systematic way. This book offers students and practitioners from all fields of mental health a clear, practical, operationalized method for constructing psychodynamic formulations, with an emphasis on the following steps: DESCRIBING problems and patterns REVIEWING the developmental history LINKING problems and patterns to history using organizing ideas about development. The unique, up-to-date perspective of this book integrates psychodynamic theories with ideas about the role of genetics, trauma, and early cognitive and emotional difficulties on development to help clinicians develop effective formulations. Psychodynamic Formulation is written in the same clear, concise style of Psychodynamic Psychotherapy: A Clinical Manual (Wiley 2011). It is reader friendly, full of useful examples, eminently practical, suitable for either classroom or individual use, and applicable for all mental health professionals. It can stand alone or be used as a companion volume to the Clinical Manual.

Perhaps the most difficult milestone in a young clinician's career is the completion of the first interview. For the typical trainee, the endeavor is fraught with apprehen sion and with some degree of dread. If the interview goes weIl, there is consider able rejoicing; if it goes badly, much consternation results. Irrespective of the amount of preparation that has taken place before the interview, the neophyte will justifiably remain nervous about this endeavor. Thus, the first edition of Diagnostic Interviewing was devoted to providing a clear outline for the student in tackling a large variety of patients in the interview setting. In consideration of the positive response to the first edition of Diagnostic Interviewing, published in 1985, we and our editor at Plenum Press, Eliot Werner, decided that it was time to update the material. However, the basic premise that a book of this nature needs to encompass theoretical rationale, clinical description, and the pragmatics of "how to" once again has been followed. And, as in the case of the first edition, this second edition does not represent the cat's being skinned in yet another way. Quite to the contrary, we still believe that our students truly need to read the material covered herein with considerable care, and once again the book is dedicated to them. We are particularly concerned that in the clinieal education of our graduate students, interviewing has been given short shrift.
