This volume contains an Open Access Chapter - This book provides a comprehensive understanding of the sustainability of health systems in Europe. Furthermore, it includes an introduction on how EU action in supporting health- care policies in the EU Member States, both looking at implemented actions and describing current priorities for the future.

This book addresses the global need for more comparative studies on health policy and health care systems, given the rise in recent decades of societal aging, modern mass diseases, economic globalization, and resulting permanent fiscal austerity of governments which have fundamentally altered the status quo of health care systems. The book examines the healthcare experiences of the most developed countries in Asia (Japan, South Korea, Taiwan and Singapore) and compares these with four of the most important health care systems in Europe (UK, France, Germany and Italy). Focusing on the public health care systems the contributors discuss the rising need for reforms in health care and health insurance administration, delivery systems, financing and overall health care policy strategies, particularly in fast-aging societies in Asia, and highly aged societies in Europe. The lack of well-documented, factual information on fraud, waste and corruption in the healthcare sector is an important ally for those who would seek to abuse healthcare systems for their own profit. Our lack of knowledge of the incidence, nature and extent of fraud, waste and corruption in healthcare is a threat to the establishment of effective counter-fraud strategies. It prevents those who finance healthcare provision from understanding in clear and quantifiable terms the need to invest resources into counter-fraud activities. As a consequence, fraud remains a matter of moral hazard and healthcare systems continue to suffer considerable financial damage, as well as all the associated consequences for the quality of care that patients receive. This volume, developed by the Observatory together with OECD, provides an overall conceptual framework for understanding and applying strategies aimed at improving quality of care. Crucially, it summarizes available evidence on different quality strategies and provides recommendations for their implementation. This book is intended to help policy-makers to understand concepts of quality and to support them to evaluate single strategies and combinations of strategies.

Countries in Europe have long recognized that good health care, though essential, is not in itself sufficient to improve health or to reduce the increasing gaps in health status between the rich and the poor. In 1984, together with WHO, they adopted and many attempted actively to implement, what has become known as the health for all policy. This called for a radical shift from health services planning to an approach based on setting objectives and targets for health, requiring partnerships with industry, agriculture and commerce and settings such as workplaces and schools. It also required changes in behaviour and action to ensure a fairer distribution of the determinants of health, such as income, education, employment opportunities, and adequate food and housing. This volume provides a broad overview of how the 51 countries in the WHO European Region set about this ambitious task. The study's findings are based on replies to checklists sent to the Member States, a set of country profiles (given as an annex), published policy documents, and case studies written by people who were involved in the process in their own countries or regions. Examining how countries used some of the policy instruments open to them as they strove to close the health gaps, the study explores how far the rhetoric was accompanied by the necessary action and whether countries were able to move from their traditional ways of working, and poses questions as to whether this approach will be sufficient to meet the challenges of the future.
