Le symptôme "vertige" varie d'un patient à l'autre et connaît de nombreuses causes. Sa prise en charge par l'ORL est spécifique, et on ne peut pas se passer de son bilan, en particulier lorsque le vertige est isolé, réduit au seul symptôme ou s'intègre dans une pathologie vestibulaire périphérique. De plus, il est essentiel pour le kinésithérapeute afin de guider sa rééducation vestibulaire. Dans le diagnostic, la vidéonystagmoscopie et la vidéonystagmographie ont été un progrès décisif : chacun peut dorénavant voir, enregistrer et revoir des nystagmus que seulement quelques prophètes avaient pu observer au siècle dernier. Par ailleurs, grâce aux progrès de l'imagerie, les tableaux cliniques répertoriés au fil des décennies se sont peu à peu clarifiés y compris aux urgences. La migraine a également pris une part importante dans le diagnostic étiologique, à côté des VPPB. Cet ouvrage très illustré est résolument pratique. L'auteur y aborde dans un premier temps les aspects anatomo-physiopathologiques, pour mieux comprendre les nystagmus et mouvements anormaux observés dans la pathologie de l'oreille interne et de l'appareil vestibulaire. Puis une partie est dédiée à la clinique, de la première consultation au diagnostic étiologique. Enfin, la 3e partie traite de la réhabilitation des patients, avec notamment un catalogue d'exercices vestibulaires.Tous les éléments pour traiter et rééduquer le patient atteint de vertige. - Les aspects anatomo-physiopathologiques, pour mieux comprendre les nystagmus et mouvements anormaux observés dans la pathologie de l'oreille interne et de l'appareil vestibulaire. - La clinique : de la première consultation au diagnostic étiologique. - La réhabilitation des patients, avec notamment un catalogue d'exercices vestibulaires.

Les causes de vertige, défini comme une illusion de mouvement, peuvent être graves : syndrome de Wallenberg, dissection de l’artère vertébrale, tumeur de la fosse postérieure et toutes sortes de diagnostics à ne pas manquer. Plus souvent ce sont des entités spectaculaires mais qui gardent encore une part de mystère : vertige positionnel paroxystique bénin (VPPB), névrite vestibulaire, maladie de Menière et migraine. Mais la plupart des patients nomment vertige, un déséquilibre ou des malaises variés. Les anglo-saxons possèdent des mots pour décrire ces symptômes : dizziness, space and motion discomfort. En français, on pourrait les traduire par « pseudoébriété », comme si on avait bu. Il faut prendre ces patients au sérieux. Comment savoir s’il manque 50 cellules ciliées dans telle macule otolithique ou telle cupule de canal semi-circulaire. Ou encore comment savoir ce qui se passe dans des réseaux de neurones comme celui du stockage de vitesse. Les bilans otoneurologiques classiques restent très souvent normaux, et le patient arrive avec une IRM normale à la main. Tout repose alors sur le bilan fonctionnel. L’électronique a progressé dans ce domaine. On mesure les gains canal par canal (VHIT). On reproduit chaque syncrétisme statique ou cinétique (masques de réalité virtuelle (RV)). La kinésithérapie à mains nues garde sa valeur, mais des rééducations par acuité visuelle dynamique (AVD) offrent des protocoles adaptés à chaque cas particulier. "More than 60 percent of global production of smoked fishery products occurs in Africa and Asia, highlighting their tremendous significance in food and nutrition security, as well as livelihood support in these regions. However, prevailing processing technologies present critical challenges, especially in terms of safety and quality aspects, and thus there is an urgent need for the development of more efficient and safer systems. The FAO-Thiaroye fish processing technique (FTT) was developed to address these challenges by yielding products that comply with international limits on polycyclic aromatic hydrocarbons in particular, while fostering many social, economic and environmental benefits. This document examines the conditions in which improved fish smoking systems in general, and the FTT in particular, have been adopted in the context of benefits, trade-offs and policy implications in selected developing countries. In fact, experience from some African and Asian countries points to the need for a context-driven balance that ensures that the gains associated with the use of this new technique can be realized without making expensive compromises, especially in terms of fisheries resources status and trade dynamics. Policy and regulatory frameworks need to be informed by a risk-based approach and supportive of consistent benchmarking and differentiation of FTT products. This document reviews the lessons from those countries, and makes the case for a hard, evidence-based, policy backbone to safeguard the sustainable, eco-friendly supply of safe smoked (and dried) fishery products to support food security, particularly in the developing world.
