As a home-health nurse, you well know the unique requirements related to seeing patients in their homes. Unlike many specialties in the nursing profession, the home-health nurse's work setting necessarily differs every day. In fact, it often changes multiple times a day as a result of having to travel from location to location, home to home, visiting the day's assigned patients. Sorting through all of the visit details at the end of each day can be a daunting task, especially since many home-health nurses must also round out their workweek by accepting assignments from multiple agencies, all of which have their own requirements when it comes to documentation. And, of course, work is seldom complete when the actual visit is over. After-visit reports must be filed, future daily visits must be scheduled, and things like mileage and out-of-pocket expenses must be logged.This unique planner/organizer is designed to help the home-health nurse be as efficient as possible by capturing all of the details necessary for reporting while also providing prompts for conducting the best possible assessment of each patient. Logically organized, it is divided into 12 monthly segments followed by 60 blank two-page assessments. Each month begins with a two-page spread comprised of a full calendar to the left and a log of all patients seen during that particular month on the right. In addition to recording the patient's name and visit date, the nurse is prompted to record the page number that correlates to each detailed assessment that has been recorded in the Patient Assessments section that follows the monthly sections. Each monthly section also includes four lined pages for notes and four dot-grid pages for other notes and illustrations that may be necessary for clarification during the month.The Patient Assessments section begins after December's monthly section. Each individual patient visit is recorded in a two-page assessment spread that uses both check boxes and blank write-in responses to prompt the nurse for standard assessment elements: Agency Info, Visit Type, Patient Info, Vitals, Medical History, Medications, Supplies Used, Nursing Assessment (Reason for Visit, Diagnosis, Vaccinations, Wound Care, Status Assessment, Functional Limitations, Cardiac, GI, Respiratory, Activities Permitted, Necessary Precautions, Teaching Provided), DME Supplies, Care Requirements, and Notes/Follow-Up. There are 60 blank assessments, and each record includes a narrative section at the very end for capturing pertinent information not recorded elsewhere. Assessment pages are numbered so that they can be keyed back to the monthly logs that appear next to the monthly calendars.
