Based on research and candid interviews with some well-known leaders whose costly blunders have made headlines, this book examines seven common mistakes, debunks myths surrounding mistake recovery, and identifies seven strategies for rebounding from near-fatal errors.

On his 81st birthday, without explanation, Karen Fisher-Alaniz's father placed two weathered notebooks on her lap. Inside were more than 400 pages of letters he'd written to his parents during WWII. She began reading them, and the more she read, the more she discovered about the man she never knew. They began to meet for lunch every week, for her to ask him questions, and him to provide the answers. It was through this process that she discovered the secret role he played in WWII. Karen's father was part of a small and elite group of men who were trained to copy and break top-secret Japanese code transmitted in Katakana. Through this journey, with painful memories now at the forefront of his thoughts, Karen's father began to suffer, making their meetings as much about healing as discovery. Thus began an unintended journey—one taken by a father and daughter who thought they knew each other—as they became newly bound in ways that transcended age and time.

In August 2018, UNSW Arts and Social Sciences, Women's Legal Service NSW and the National Centre for Indigenous Excellence sponsored and hosted a unique forum of 42 Aboriginal, Torres Strait and Pacific Islander women intended to 'Break Silent Codes' and share stories of cultural and spiritual responses to the issue of sexual assault and family violence in communities across Australia and the Pacific. The forum provided a platform for women to discuss the ways in which community, religion, authority and family create silences around sexual assault and family violence. There are many injustices experienced by Aboriginal and Torres Strait and Pacific Islander communities. For a long time, it has seemed that all other matters of injustice are more important than the sexual assault and domestic violence in communities. Women experience this as a silencing of issues important to their spiritual and physical well being. Through this book, we share with you the stories of this gathering which has now become a movement of its own for First Nations women across Australia and the Pacific.

Collins Award for Best Book of Nonfiction In the age of #MeToo, learn how brave whistleblowers have dared to lift the federal court’s veil of secrecy to expose powerful judges who appear to defy laws they have sworn to uphold Code of Silence tells the story of federal court employee Cathy McBroom, who had to flee her job as a case manager in Galveston, Texas, after enduring years of sexual harassment and assault by her boss—US District Judge Samuel Kent. Following a decade of firsthand reporting at the Houston Chronicle, investigative reporter Lise Olsen charts McBroom’s assault and the aftermath, when McBroom was thrust into the role of whistleblower to denounce a federal judge. What Olsen discovered by investigating McBroom’s story and other federal judicial misconduct matters nationwide was shocking. With the help of other federal judges, Kent was being protected by a secretive court system that has long tolerated or ignored complaints about corruption, sexism, and sexual misconduct—enabling him to remain in office for years. Other powerful judges accused of judicial misconduct were never investigated and remain in power or retired with full pay, such as US Circuit Judge Alex Kozinski and Kozinski’s mentee, Brett Kavanaugh. McBroom’s ultimate triumph is a rare story of redemption and victory as Judge Kent became the first and only federal judge to be impeached for sexual misconduct. Olsen also weaves in narratives of other brave women across the country who, at great personal risk, have reported federal judges to reveal how sexual harassment and assault occur elsewhere inside the federal court system. The accounts of the women and their allies who are still fighting for reforms are moving, intimate, and inspiring—including whistleblowers and law professors like Leah Litman, Emily Murphy, and novelist Heidi Bond, who emerged to denounce Kozinski in 2017. A larger group of women—and men—banded together to form a group called Law Clerks for Accountability, which is continuing to push for more reforms to the courts’ secretive complaint review system. Code of Silence also reveals the role the press plays in holding systems of power in check. Kent would not have been charged had it not been for Olsen’s reporting and the Houston Chronicle’s commitment to the story.
