The rigid internaI fixation of mandibular fractures has become a widely ac cepted practice among European surgeons. The caution or even outright re jection voiced at a congress of the German Society of Maxillofacial Sur 1970s is no longer prevalent. Through a process of geons held in the late critical review and implementation, rigid internaI fixation has become an established treatment modality at numerous centers, especially in Switzer land, the Federal Republic of Germany, and the Netherlands. By comparison, the method has received very little attention in North America and the Anglo-Saxon countries. By and large, surgeons in these countries continue to treat mandibular fractures by intermaxillary fixation, possibly supplemented by the use of interosseous wires. Many recent edi tions of surgical texts confirm this. Lately, however, there appears to be a surge of interest in methods of functionally stable internaI fixation, especially in the United States of America, and AO/ ASIF instruction courses are increasingly in demand. This book is intended to aid course participants in their lessons and practi cal exercises and also to guide the clinical practitioner in the application of AO/ ASIF principles. Since 1958 the Association for the Study of Internal Fixation (ASIF) has worked to establish the experimental and biomechanical prin ciples of the operative treatment of fractures. It has been proven that immediate, pain-free mobilization, made possible by stable internal fixation, is the key to restoring function in a fractured extremity. The same principles were applied in 1972173 by Roland Schmoker, who developed a universal plate and sophisticated instrument set designed initially for the treatment of comminuted fractures of the mandible. To extend the applications of his plate to reconstructions after ex tensive tumor resections, he supplemented the device with tem poromandibular joint prostheses and anchoring elements for the at tachment of dentures. He first tested the functional stability of these implants experimentally in sheep. Later he found that the minipig had a more human-like jaw shape and masticatory action, and so he tested the implants in 37 mini pigs that underwent extensive mandibular re sections without bone grafting, using sequential dye injections and postoperative weight gain to chart the progress of osseous regener ation. Seven other animals were not operated and served as controls.

Functionally stable internal fixation is of particular relevance to maxillo facial surgery, because it obviates the discomforts and inconveniences of intermaxillary fixation. Given the biomechanics and biophysics of the skeletal system, the true im mobilization of bone can be achieved only through highly technical means. Willenegger speaks of an "advanced school" of bone surgery which, when fully realized, will enable excellent results to be achieved even in the most difficult fractures. To accomplish this goal, ongoing refinements are needed in surgical methods and technology. Advancing the state of operative tech nique has been a central concern of the Association for the Study of Inter nal Fixation since its establishment 25 years ago. For this reason, a major priority of the AOI ASIF has been to develop its own surgical instrumen tation. With the help of technical commissions comprised of experts from medi cine, research and manufacturing, the AOI ASIF has been able to develop and successfully test a line of surgical instruments whose trademark is known and respected the world over. For every specialty in traumatology and orthopaedics, including maxillofacial surgery, the AOIASIF has devel oped both a basic and a special instrument set designed to meet specific anatomic requirements.
