Among the most recently developed procedures for treating heart disease in a minimally invasive way are the techniques for percutaneous treatment of valvular heart disease. These are important advances because the only alternative is open heart surgery, which is not an option for many high-risk patients. This issue provides a detailed description of several percutaneous procedures, including trans-catheter aortic valve implantation.

A comprehensive survey of nonsurgical treatment for a variety of heart diseases that affect the cardiac valves, the heart muscle, and the structure of the heart. The authors describe who these procedures are useful for, how to do them, and how well they work. Major topics of discussion include percutaneous techniques for valvular heart disease, septal defects at both the atrial and ventricular levels, adjunctive therapies during coronary interventions, and angioplasty to treat extracardiac vascular disease, as well as reviews of the cutting-edge imaging modalities now being used in interventional procedures. An accompanying CD-ROM contains video demonstrations of catheterization and and the imaging portions of these procedures.

Over the last three decades, major advances in the management of diseases of the valves have been seen. While cardiac catheterization was in the 1970s, the only means for con?rming the diagnosis, Doppler echocardiography has become the corner stone for accurate assessment of valve disease, even in fetuses. Now not only the disease can be identi?ed but also the rate of disease progression (e. g. lea?et calci?cation and stiffness) can be accurately measured and quanti?ed by echocardiography and multislice/electron beam CT imaging. Surgical treatment of valve disease has also w- nessed great advances, having moved from traditional valve replacement to sophisticated repair procedures and the use of extracorporeal pump support in some patients. Robotic valve surgery has also proved a great success, in well selected cases, and is expected to have a fruitful future. Furthermore, in special circumstances, conventional surgical valve excision has now been replaced by percutaneous catheter-based valve replacement for aortic and pulmonary disease. This approach reduces the risk from the surgery itself, avoids general ana- thesia and many potential post-operative complications. This book discusses the practicalities of the diagnosis and treatment of the various aspects of common heart valve d- eases, covering most clinical and surgical issues. It is designed to assist clinicians in the management of patients with valve disease and provide them with answers to many of the clinical questions that arise.

The only reference wholly dedicated to this complex topic, Diagnosis and Treatment of Mitral Valve Disease: A Multidisciplinary Approach provides authoritative, must-have information on the disease processes affecting the mitral valve, interpretation of diagnostic studies, and medical, transcatheter, and surgical treatments needed for mitral valve repair. Using a multidisciplinary approach, Drs. Scott Goldman and William Gray oversee a “who’s who team of contributing authors who offer a holistic view of the modern treatment of mitral valve disease for today’s cardiac surgeons, cardiologists, and other cardiac team members. Includes foundational chapters on anatomy and pathophysiology of the mitral valve—critically important topics when treating multiple complex and highly dynamic structures that function as a single unit. Covers diagnostic tools, medical treatments, surgical therapies, and transcatheter therapies, and provides tips for developing a successful mitral valve program (with baseline thresholds, common pitfalls, and a checklist for hospitals). Uses an accessible, quick-reference format that features takeaway chapter openers, bulleted key points, end-of-chapter summaries, a common nomenclature and glossary of terms, key references, and ample line drawings, photographs, tables, charts, and graphs throughout. Incorporates the essential role of imaging in structural transcatheter interventions, including echocardiography and transcatheter edge-to-edge repair (TEER), and their uses in diagnostic work-up, procedural planning, intra-procedural guidance, and follow-up. Features dozens of procedural videos, including transthoracic echocardiogram; cardiac MRI for mitral regurgitation; transcatheter mitral valve repair; transcatheter valvuloplasty; percutaneous mitral valve interventions; MitraClipTM positioning; and many more.
