Strengthening Health System Governance Better Policies Stronger Performance

Governance is the systematic, patterned way in which decisions are made and implemented. The governance of a health system therefore shapes its ability to respond to the various well-documented challenges that health systems face today, and its capacity to cope with both everyday challenges and new policies and problems. This book provides a robust framework that identifies five key aspects of governance, distilled from a large body of literature, that are important in explaining the ability of health systems to provide accessible, high-quality, sustainable health. These five aspects are transparency, accountability, participation, organizational integrity and policy capacity. Part 1 of this book explains the significance of this framework, drawing out strategies for health policy success and lessons for more effective governance. Part 2 then turns to explore eight case studies in a number of different European regions applying the framework to a range of themes including communicable diseases, public-private partnerships, governing competitive insurance market reform, the role of governance in the pharmaceutical sector, and many more. Ensuring a robust and resilient health system involves policy actions which need to be implemented based on the best available evidence. This requires health systems to be monitored regularly to build on their strengths and to overcome any apparent shortcomings. In order to assist in that process, this volume, a collaboration between the World Health Organization and the European Observatory on Health Systems and Policies, presents a new framework to support monitoring of health system performance, with a focus on detailed conceptual links between health system functions and overall system goals. This HSPA framework for Universal Health Coverage thus represents a comprehensive attempt to address fundamental questions regarding regular assessment of health systems, including health system boundaries, component elements and outcomes. In this book, each of the health system function chapters outlines the purpose of the function, the sub-functions that enable it to carry out the key activities necessary to fulfil its purpose, as well as the assessment areas and proposed indicative measures to evaluate how well a system performs. The framework will thus assist policy-makers in understanding possible origins or impact of poor performance on a particular health system outcome, triggering more in-depth analysis.

Factors outside of healthcare services determine our health and this involves many different sectors. Health for All Policies changes the argument about inter-sectoral action, from one focusing on health and the health sector to one based on co-benefits – a 'Health for All Policies' approach. It uses the Sustainable Development Goals as the framework for identifying goals across sectors and summarizes evidence along two causal axes. One is the impact of improved health status on other SDGs, e.g. better educational and employment results. The other is the impact of health systems and policies on other sectors. The 'Health for All Policies' approach advocated in this book is thus a call to improve health to achieve goals beyond health and for the health sector itself to do better in understanding and directing its impact on the world beyond the healthcare it provides.
